Community Based Supports provide assistance for individuals who need direct support to achieve independence and inclusion.
Services are delivered one-on-one by trained staff and may include:
- assistance with community based activities
- assistance to perform various tasks like self-care, social skills, and daily living activities
This may include assisting the member attend an event, take a class, work on daily living skills, order off a menu, purchase items, learn basic cooking and laundry skills, etiquette, or accessing activities in the community.
Depending on the level of need, support may be provided anywhere from 2 – 24 hours of support per day.
