Support Coordination Agency
Welcome to CAU! This page will provide information about how we can help you plan for services offered by the NJ Division of Developmental Disabilities (DDD).
CAU has been providing support coordination type services to individuals with disabilities across the state for over 10 years. CAU has a team of over 30 highly trained staff throughout the state, in every county, who are ready to assist you and your family. We are confident that we can offer the most comprehensive support to help you decide how to plan for the needs of your loved one utilizing your individual budget and help make connections to the most valuable supports necessary. In addition, our support coordinators will help you access other services that may help to supplement/complement the services you purchase with your individual budget. All other services suggested will be based on individual circumstances and eligibility factors.
CAU support coordinators will assist you in discovering all the potential of your loved one, how supports and services can be utilized to reach that potential, how to develop your individualized plan, coordinate and monitor your plan and services. During a series of initial meetings, telephone calls and exchanges, CAU support coordination will assist you in utilizing and coordinating community resources, other programs/agencies to develop your plan. Natural supports of family, neighbors and community will be explored. Programs and services by local, state and federal agencies will be examined. CAU support coordinators will provide linkage and coordination with all appropriate supports. This will result in your unique service plan that will become the blueprint for the future.
Once your plan is developed and approved by DDD, CAU support coordinators will continue to be there every step of the way. We will help you make revisions as necessary or as circumstances change. You can expect monthly contact, quarterly face-to-face visits and an annual home visit.
CAU support coordination operates 7 days a week, so we are available when you are to assist you in this process. CAU support coordination offers a 24 hour hot line in the event of an emergency.
All CAU support coordination have at minimum a bachelor’s degree, at least one year experience in the field, completed all DDD required trainings and have been verified though a process of state and federal criminal background checks and checked through the Central Registry. Support coordination services are available in English, Spanish, Chinese and other languages upon request.
