The following programs at CAU are designed to benefit senior citizens, veterans, or individuals living at home who are looking for support services that help them maintain their independence.
CAU’s Home Health Services offer assistance ranging from a few hours a week to 24-hour care for people living in the community looking for help to help them live independently in their home. Community Access Unlimited acts as the fiscal intermediary for the following programs:
The Veterans Direct Home & Community Based Services Program (VD-HCBS) allows veterans who are potential candidates for nursing home placement to receive in-home support for the level of care they require. CAU will assist veterans in choosing their own care providers outside of the VA health care system. In some cases, the family members who care for these veterans can be paid for the care they provide.
The Community Living Program grant initiative under the Administration on Aging (AoA) is designed to assist individuals who are at risk of nursing home placement and help to manage their Medicaid budget so that these individuals can live at home in their community. CAU is a community provider in this instance that will work with those in the CLP program to receive the appropriate services and live independently.
